PARTICIPATION REQUIREMENTS
What Is Needed for Medicaid Home-Care Support?
Key participation requirements for CCSP ( Community Care Services Program ) and SOURCE home-care services include:
- Live in GeorgiaThe person receiving care must be a resident of the state of Georgia.
- Meet Medicaid eligibility requirementsMedicaid eligibility must be reviewed as part of the participation process.
- Need a nursing-facility level of careThe person must have care needs that would otherwise require support in a nursing facility.
- Need daily support at home or in the communityCare needs may include help with personal hygiene, meals, mobility, and other daily activities.
Services are provided according to individual needs and an approved care plan.
